NSAA Office Notice

May we request you to accomplish the profile forms not later than 28 February 2009.  Non submission of the following may mean withholding of allowance.

• fully accomplished NSA Affairs form -1 to be encoded in enclosed disc.
• all choiced-boxes in NSA affairs form 1 have to be filled with red ink
• photocopy of passports or NSO issued birth certificate
• 1 passpost size latest photo
• specimen signature would have to be signed in the presence of an NSA Affairs Staff Member at the NSA Affairs Office.

By way of information, starting March 2009, ALL bank accounts of national athletes and coaches for payroll purposes will be transferred to LANDBANK PHILIPPINES.
